  • Reside in San Mateo or Santa Clara County*
  • Patient at LPCH or affiliated clinics, including Ravenswood Family Health Center
  • Family income under 250% of the Federal Poverty Level*

    • *Residents from all counties and all income levels will receive appropriate referrals.
